Federally Qualified Health Centers (FQHCs) can be an important resource for students and families who need help accessing follow-up care.
 
FQHCs provide healthcare services to patients regardless of insurance status and offer sliding-fee programs based on a family’s ability to pay. Many locations provide multiple services, including primary care, dental and behavioral healthcare.

Funding Assistance

Help Families Access Recommended Care

When cost or insurance coverage prevents a student from receiving recommended follow-up care, First Hand Foundation may be able to help.
 
Our Individual Medical Grant program provides funding assistance for eligible healthcare needs for children, including medical treatment, equipment and other medically necessary care.
 
School nurses are encouraged to share this resource with families who may need financial assistance completing recommended follow-up care.
 
 
Important: Families should apply for funding assistance before receiving services whenever possible, as First Hand generally cannot reimburse families or providers for care that has already been provided.

REFERRAL FOLLOW-UP PROCESS

 

School nurses are an important partner in helping students receive recommended follow-up care after a Healthe Kids screening.

Review

First Hand provides the school health office with screening results and information about students who received follow-up recommendations.

Support

Connect with families as appropriate and help them understand recommended next steps or identify barriers to care.

Follow Up

Check in with the family to determine the current status of the student's referral.

Update First Hand

Submit the student's current referral status through our secure Referral Follow-Up Form.

 

You don’t have to wait until care is completed to update us. Tell us if an appointment is scheduled or completed, follow-up is already in place, the family is experiencing a barrier, the family has chosen not to follow up, or another circumstance applies.
